 You only need to complete one form, Application or Nomination. (Not both)

 To Apply Yourself:

  1. Fill out the Application Form

  2. Write an Essay

  3. Attach a letter of recommendation completed by your Faith Formation Coordinator or Priest.

 To Nominate a Young Woman:

  1. Fill out the Nomination Form

  2. Attach a letter of recommendation completed by your Faith Formation Coordinator or Priest.

  3. No Essay is required.

 Recipient Receives:

  • Registration for her and a guest to attend our ACCW Convention

  • Individual ACCW Membership

  • Golden Rose Rosary

 One winner from each Deanery. (7 Total)
